Bilingual Case Manager
Job Summary: Case Management services are offered to residential clients in both our Emergency Shelter and Transitional Housing programs, as well as to community members on an outpatient basis. The focus of case management services is to establish safety and assist survivors of domestic and sexual violence to enhance their stability. This is achieved by offering tangible support and active advocacy to assist survivors to meet their individual needs and to identify goals that are meaningful to them. The Case Manager works as a hub to coordinate and facilitate access to services with multiple internal and external resources and referrals. Candidates should anticipate the need to actively manage their time and assigned tasks to be able to provide trauma informed client services while promoting case progression amidst competing priorities.
Identify and prioritize victim’s needs, including safety planning and danger assessment
Develop goals and objectives specific to the victim’s own goals and record these in a program-approved service plan targeted towards self sufficiency
Identify resources available to victims and facilitate internal and external referrals to assist in goal/objective achievement
Provide encouragement and support towards completion of victim’s goals and objectives
Evaluate and plan adaptation to victim’s changing needs, as appropriate
Conduct follow-up services with clients after their residential stay
Work creatively to navigate systemic and/or motivational barriers that might affect self-sufficiency
Develop creative solutions that provide opportunities based on individual needs
