Job Summary 

The Case Management Supervisor will provide trauma-informed, guest-driven case management services to individuals experiencing homelessness at The Road Home’s Family Interim Non-Congregate Children Housing (FINCH) crisis services and shelter program with the goal of helping them achieve stable housing solutions quickly and efficiently. The Case Management Supervisor will supervise a team of Case Managers through client interactions, administrative needs, and other duties, including but not limited to maintaining professional boundaries, emphasizing teamwork, and creating an environment that fosters growth. The services provided include identifying and engaging with individuals at FINCH and connecting them with resources in the community that will aid in their transition into housing. The Case Management Supervisor will work within the shelter team to identify common barriers faced by individuals in shelter and help with establishing strategies and procedures for addressing needs on a shelter-wide scale.

Supervision 

  • Provide direct supervision and ongoing evaluation for a team of Case Managers. 
  • Evaluate and hire new CMs. 
  • Assist staff with setting strengths-based, achievable goals targeting quick and stable housing solutions for their case load. 
  • Support and train staff in providing thorough and individualized support based on each guest’s unique needs and barriers, maintaining focus on Housing First approaches to sheltering and supporting individuals back into housing. 
  • Work with staff to develop new and innovative ways to engage with guests. 
  • Timely review and approval of staff timesheets. Manage other departmental paperwork and administrative reports in a timely manner. 
  • Assist SSM in managing weekly staff schedule and ensure each shift is adequately staffed. 
  • Take the lead in assuring the case management team is following through with prioritizing clients through weekly Family Progressive Engagement (FPE) meeting. 
  • Manage the Lethality Assessment Program (LAP) spreadsheet and necessary follow up. 
  • Ensure best practices are implemented and adhered to: 
  • Housing First 
  • Harm Reduction 
  • Trauma-Informed Care 

Housing First   

  • Help individuals experiencing homelessness get into housing.  
  • Assist guests in identifying barriers and connect them with appropriate resources in the community.  
  • Celebrating and assisting our guests and their belongings move into their new homes. *  
  • Use agency vehicles to provide transportation for guests in their pursuit of housing. *  
  • Strong belief of the Housing First model. Willingness to initiate and have ongoing conversations with guests about housing.  

Managing a Case Load  

The Case Management Supervisor will case manage a small caseload of 3-5 clients while engaging in the following… 

  • Assisting our diverse population of guests on your set caseload to establish goals.  
  • Engaging with all guests to connect them with resources and housing options.  
  • Following agency guidelines to create healthy boundaries with guests.  
  • Daily collaboration with all departments promoting a healthy team environment.   
  • Seeking out, engaging, and networking with community partners to find housing options and additional resources.  
  • Attending daily open office hours as a team to interact and provide guests with resources to further their housing goals.   
  • Attending weekly team meetings and contributing to case conferencing conversations.  
  • Keeping accurate records, case management notes, and statistical data as required and participate in special projects as needed.  

Crisis Management 

  • Provide direct, trauma-informed case management services to guests, including crisis intervention, harm reduction, outreach, ongoing assessments, goal setting, creative problem solving, using a network of community resources.   
  • Utilizing the training we provide to jump into high stress situations in an effective and trauma-informed way  
  • Assist with de-escalation and crisis intervention with guests in crisis.  
  • Collaborate with the crisis clinician and others to find ways to support individuals with mental and behavioral health barriers.  
  • Ability to accept direction and constructive feedback from Supervisor.  
  • Ability to set and maintain good boundaries, using Trauma-Informed Care as a guideline.

  • Ability to contribute to the Agency’s commitment to enhancing awareness and appreciation  
  • Willingness to understand that housing first, harm reduction, and low barrier access to resources is best practice and essential to our mission at The Road Home.

Education and Experience 

  • Lived, work or volunteer experience preferred or a degree in a related field  
  • Experience working or volunteering with diverse populations  

Required Skills and Abilities 

  • Excellent writing and documentation skills. 
  • Willing to be a proactive member of a team. 
  • Ability to work with community partners. 
  • Self-starter and self-directed. 
  • Ability to accept direction from Supervisor. 
  • Excellent organizational skills and strong computer skills. 
  • Ability to work from a Trauma-Informed Care framework.  
  • Familiarity with the Housing First philosophy.  
  • Demonstrated awareness of and sensitivity to diverse populations. Ability to contribute to the agency’s commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.  
  • Must pass a pre-employment background check and drug screen. 

Physical and Equipment Requirements 

  • Ability to lift 15 pounds.  
  • Ability to stand and walk for at least an hour at a time.  
  • Ability to drive a vehicle.
