First Step House is hiring a full time Recovery Residence Case Manager in Salt Lake City, Utah!!

The Recovery Residence Case Manager serves clients that are living in First Step House's recovery residence sober living. The goals of this program are to (1) provide housing case management services in a sober living setting (2) develop individualized service plans with clients that focus on finding permanent housing and (3) connect clients with community resources and sober supports. The role of the RRCM is to assist the recovery residence manager with client check-ins and daily facility checks, complete individualized case plans with clients, assist clients with finding permanent housing options in the community and provide additional case management services to help them remain stable in recovery residence.

  • Pay Range: $ 44,000 - $ 46,000 BOE
  • Schedule is THURSDAY-MONDAY Day Shift!

Requirements:

  • Daily and exemplary attendance and punctuality is required for this position
  • Must have strong interpersonal skills. This includes the ability to listen, give direction and guidance to clients, and work collaboratively with counselors and other case workers to provide effective case-management services
  • Must have the ability to negotiate difficult situations and help motivate clients to better their circumstances
  • Must have the ability to effectively read and write business letters, procedures, reports, and more
  • Must be organized and able to follow instructions accurately and precisely
  • Must have the ability to learn and use technological tools including databases, excel spreadsheets, and more

Education and Experience

  • Case management experience is preferred
  • Experience in the substance-use/recovery field is preferred
  • A minimum of a bachelor's degree in social work or related field is preferred

Duties include but are not limited to:

  • Daily check-ins and on-site residence checks
  • Addresses tenant concerns when Recovery Residence Manager is unavailable
  • Assists Recover Residence Manager with intakes, move outs and turnover of rooms
  • Perform needs assessments with clients
  • Develop individual case management plans with each client and meet regularly to review progress
  • Collaborates with outpatient counselors, case workers, and outside agencies to coordinate services
  • Completes Housing Action Plans (HAP) with tenants to set goals for permanent housing
  • Assist clients/tenants with moving out to permanent housing, as able
  • Collaborates with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources
  • Uses internal tracking system to ensure proper, accurate, and timely billing
  • Uses KIPU database and Homeless Management Information System (HMIS) to enter case-management notes, client information updates, billing information and other relevant client information
  • Gathers appropriate releases of information to ensure compliance with both the Federal Health Insurance Portability and Accountability Act (HIPAA) and the Code of Federal Regulations (CFR 42 and 45)
  • Completes all treatment-related letters, documentation, internal processes, and data entry in a timely manner
  • Maintains organizational expectations for productivity
  • Attends all relevant employee training classes and staff meetings
  • Maintains knowledge of program rules, client rules, client rights, and relevant legal and ethical codes
