Case Manager
Job Summary: The Case Manager is responsible for working with a caseload of individuals and/or families as assigned by the Clinical Director. The Case Manager works closely with each client to address the basic needs of clients; and provides individualized case management and referrals to help clients achieve self-sufficiency. The Case Manager is an active and integral member of the Clinical Team.

Physical Demands: While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to sit (at a desk for extended periods of time), stand, and walk. This position requires the ability to occasionally lift office products and supplies, of up to 10 pounds. | |
Travel: A valid driver’s license and current automobile insurance are required. You may need to transport clients occasionally. | |
