Case Manager - REACH
First Step House is hiring a full time Case Manager at our REACH location in Salt Lake City, Utah!

The REACH case manager is responsible for overseeing our case management program for REACH. The REACH case manager helps individuals involved in the criminal justice system who have been identified as high risk for recidivism and have moderate to severe substance use disorders with case management needs. This position supports the maintenance and development of stable relationships between couples and families by enhancing housing, educational, and employment stability. This involves both direct and indirect services including goal setting, needs assessments, tracking client progress, and connecting clients with a range of resources (e.g., education, employment, medical, and housing). The REACH Case Manager helps clients learn work-related skills, create budgets, develop educational and career plans, find and maintain safe and affordable housing, and learn basic life skills that enhances a client's ability for future success. This role requires strong collaborative skills, as case managers work with various teams, including clinical and legal entities, and has a myriad of direct case management services provided to clients. The REACH Case Manager Supervisor assists the REACH Clinical Program Manager in other administrative tasks and works in concert with the priorities of the Executive Director, the Senior Management Team, and the Board of Directors. The overall goals of this position are to (a) provide high-quality care for clients, stakeholders, and team members, (b) coordinate case management services for the REACH program, and (c) perform management responsibilities to support the case management and administrative teams.

- Pay Range: $ 46,353.60

Duties to include but not limited to:
- Develops and performs assessment instruments and/or tools with clients
- Develops case management plans with each client and meets regularly to review progress
- Teaches life-skill classes and any other relevant classes that target criminogenic risk behaviors
- Assists in hiring of relevant staff for REACH
- Coordinates with outside agencies to provide psycho-educational classes to clients
- Collaborates with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources
- Collaborates with counselors, case workers, and outside agencies in the delivery of case management services
- Provide direct case management services ( 50+ caseload), including housing case management for homeless individuals
- Client engagement
- Manage voucher program to provide recovery support services to clients
- Manage data, and submit timely outcome reports to various funding sources
- Uses the Utah Web Infrastructure for Treatment Services (UWITS) database to enter case-management notes, client information updates, billing information and other relevant client information
- Gathers appropriate releases of information to ensure compliance with both the Federal Health Insurance Portability and Accountability Act (HIPAA) and the Code of Federal Regulations (CFR 42 and 45)
- Completes all treatment-related letters, documentation, internal processes, and data entry in a timely manner
- Utilizes other software programs as needed (e.g., Microsoft Word, Excel)
- Completes TPS reports monthly and meets with supervisor to regularly discuss performance based job duties.
Education and Experience:
- Case management experience is required
- A minimum of a Bachelor's degree in social work or related field is preferred
- Experience with supervision and leadership is preferred
