Case Manager - TBRA
We are hiring for a full time Case Manager in Salt Lake City, Utah!!

The Case Manager - TBRA reports directly to the Case Management Supervisor and the HR Director.
The TBRA Case Manager serves clients who receive tenant based rental assistance through TBRA funding at First Step House. The goals of this program are to (1) provide short – mid-term rental assistance for individuals and families under 60% AMI who are at risk of homelessness (2) Develop and assist in executing a Housing Action Plan (HAP) to eliminate barriers and obtain housing (3) Utilize progressive engagement to tailor services to the needs of the household in order to assist them in maintaining permanent housing. The role of the TBRA case manager is to conduct assessments, determine eligibility, develop individualized case plans, link clients with community-based housing and service resources, manage financial assistance funds, and provide case management services. The TBRA Case Manager assists in other administrative tasks as needed and works in concert with the priorities of the Executive Director, the Senior Management Team, and the Board of Directors.

- Pay Rate $45,445 per year
- Monday - Friday flexible hours

Education and Experience
- Housing case management experience is required
- A minimum of a Bachelor's degree in social work or related field is preferred
- Experience in the substance-abuse field is preferred
Duties include but not limited to:
- Screen clients for program eligibility
- Gather documentation for household's income
- Develop and execute Housing Action Plan
- Assist eligible clients with the housing process – applications, moving in, setting up utilities etc
- Determine and recommend to supervisor the level and duration of financial assistance for eligible participants
- Support clients as they navigate the housing process, including but not limited to housing location, landlord advocacy, rental application, credit repair support, move in support and budgeting
- Develop close working relationships with landlords, local service providers, and collaborative agencies
- Develop case management plan with each client and meet regularly to review progress
- Utilize progressive engagement to tailor services to the needs of the household
- Collaborate with outside agencies to connect clients with employment services, education resources, healthcare programs, financial programs, and other relevant community resources
- Collaborates with counselors, case workers, and outside agencies to deliver case management services
- Performs regular individual case-management sessions
- Interface with landlords as necessary to mitigate issues or advocate for program participants
- Prepare requests for financial assistance and submit all required documentation to supervisor for approval
- Manage and track financial assistance funds
- Reassess eligibility and supportive service needs throughout assistance until program exit
