Client Care Coordinator
The Client Care Coordinator plays a pivotal role at The INN Between (especially during our expansion!), overseeing the delivery of high-quality, integrated social services for residents. This position focuses on coordinating medical and social care, and ensuring a seamless intake and case management process. The role also emphasizes collaborative partnerships, rigorous documentation, crisis management, and continuous quality improvement to meet the evolving needs of residents.
RESPONSIBILITIES
- Care Coordination: Coordinate intakes for new residents, ensuring thorough assessments and the development of individualized case and goal plans. Oversee the implementation of these plans, ensuring they reflect a strengths-based approach, incorporate Housing First principles in supporting residents with housing applications, and address both social and medical needs through medical case management. Work collaboratively with the Administrator to manage referrals and admissions, ensuring smooth transitions into the program. Provide oversight for room checks, ensuring rooms are clean and organized. Oversee resident transportation. Facilitate housing assessments (SPDATs).
- Collaboration and Integrated Care: Coordinate closely with internal medical staff, mental health professionals, substance use counselors, and external service providers to ensure residents receive holistic, integrated care. Participate in interdisciplinary team meetings to review client progress and adjust care plans as needed.
- Documentation and Reporting: Maintain complete, timely, and accurate records in The INN Between’s systems (UHMIS, EMR), including client notes, care plans, progress updates, and required reports. Ensure all documentation complies with regulatory, funding, and program requirements. Work toward achieving HMIS Security Officer certification to strengthen data security and compliance within the organization.
- Crisis Management: Provide leadership and guidance in responding to behavioral emergencies, ensuring resident and staff safety. Offer on-call support for night and weekend staff, providing expert advice and intervention when urgent issues arise.
- Quality Improvement: Actively participate in program evaluations and quality improvement initiatives. Identify areas for service enhancement and assist in the design and implementation of improvements to increase program effectiveness and resident outcomes.
- Compliance: Ensure adherence to all relevant regulations, policies, and procedures governing social service provision and confidentiality.
REQUIREMENTS/QUALIFICATIONS
- Ability to operate within our core values of respect, kindness, honesty, compassion, and dignity.
- Experience working with at-risk and/or homeless populations with compassion for the homeless
- BSW, LCSW or equivalent, a plus
- First Aid/CPR certification or willingness to obtain
- Ability to learn and utilize software programs in use by The INN Between
- Ability to prioritize multiple tasks; flexibility to respond to changes in a fast-paced environment
- Ability to drive company vehicle; possess a current driver’s license and have a good driving record
- Ability to work occasional evenings, weekends, and be on-call for behavior issues
- Knowledge of Electronic Medical Record Management Systems, a plus
- Ability to pass State background check
- Recognizes time as a valuable resource and responds promptly to the needs of clients and co-workers.
- Consistently performs work assignments in a time-efficient manner. Adheres to deadlines

HEALTH REQUIREMENTS
- Health Evaluation performed by Director of Nursing, including TB test immunization review.
- Must be current on flu and COVID-19 (including booster).
