Director of Nursing
The Director of Nursing (DON) is responsible for managing, directing, and supervising the nursing and medical respite/personal care staff and services for all residents of The INN Between community. The DON serves as a liaison with community agencies and care providers.
RESPONSIBILITIES
- Provide care according to the Utah State Department of Health Rules and Regulations for Assisted Living Facilities Level II, §R432-270, and Utah Nurse Practice Act §R156-31B-701.
- Understand and comply with all Assisted Living regulations.
- Assess new residents prior to admission for appropriateness for ALF II level of care.
- Assess residents every 6 months or more frequently as needed due to significant changes in condition.
- Develop Service Plans in coordination with the resident, family, and responsible persons within 7 days of admission.
- Assure that Service Plans are properly implemented and documented by resident care staff on all shifts.
- Monitor resident quality of care daily.
- Assist residents in arranging access for ancillary services for medically related care and other services necessary to support the resident. Coordinate ancillary health and therapeutic services for residents if needed and/or requested.
- Supervise, coach, and evaluate all members of the clinical and resident support team including:
- Registered Nurse (RN)
- Medical Respite Care Coordinators
- Resident Advocates
- Certified Nurse Aides (CNAs)
- End of Life Doula
- Ensure accurate and timely submission of time cards for all supervised staff.
- Ensure accurate records of all resident care are maintained.
- Monitor staff personnel records to ensure inclusion of certifications, first aid/CPR, core competencies and orientation completion, medication delegation, food handlers, health evaluations, etc.
- Provide oversight of the Medication Management Program in compliance with regulations.
- Ensure that all appropriate communication and documentation regarding resident care issues are shared with the Administrator.
- Document incidents, medication errors, and significant changes for weekly review with the Administrator.
- Manage the Quality Assurance Plan and hold quarterly quality assurance reviews in coordination with the Administrator.
- Provide regular staff education (including monthly core competency in-service training) opportunities, including appropriate training for the administration of medications.
- Evaluate and update procedures on an ongoing basis.
- Review Employee Health Evaluations for new hires and perform TB testing.
- Foster a collaborative, interdisciplinary approach to care, promoting teamwork and communication among nursing, care coordination, and advocacy staff to ensure holistic support for residents.
- Participate in leadership meetings and strategic planning to enhance program quality and outcomes.
REQUIREMENTS
- Current Utah Nursing License (RN or APRN).
- Experience working with at-risk and homeless populations with compassion for the homeless a plus.
- Minimum of 1 year Assisted Living Facility Type II nursing experience preferred.
- Supervisory or management experience in a healthcare or community-based setting strongly preferred.
- Knowledge of Electronic Medical Record (EMR) systems.
- Flexible schedule and availability for on-call nights, weekends, and holidays.
- Ability to pass State background check.
PHYSICAL REQUIREMENTS
- Carrying and lifting up to 50 lbs.
- Hearing/listening.
- Manual dexterity, pulling/pushing, seeing, speaking, squatting/kneeling, standing, sitting, and walking.

- Health Evaluation performed at The INN Between, including TB test and review of all immunizations.
- Must be current on flu and COVID-19 (including booster).
