Evening and On-Call LPN or RN

The INN Between is Utah’s only medical respite and end-of-life care facility for individuals experiencing homelessness. We provide a safe, supportive, and dignified environment where residents receive compassionate care and stability during critical health transitions.
Position Overview
The Evening & On-Call Nurse (LPN or RN) supports resident health and safety during evening hours and overnight periods. This position provides on-call support to respond to emergencies, guides CNAs and Resident Advocates in decision-making, and ensures timely medical respite assessments and coordination of care. Working closely with the Director of Nursing (DON), this nurse helps bridge direct care and care coordination to ensure comprehensive support for medically vulnerable residents.
Key Responsibilities
Resident Assessment & Care
- Respond to evening and overnight calls from CNAs and Resident Advocates to assess resident needs and provide clinical guidance.
- Conduct medical respite assessments, create or update individualized medical respite care plans, and document all interventions.
- Assist in developing and implementing Service Plans and ensure care aligns with Utah Department of Health ALF II regulations and the Utah Nurse Practice Act.
- Monitor and evaluate resident health changes, coordinating with the DON and external providers as needed.
Care Coordination
- Collaborate with the medical respite team to integrate medical, behavioral health, and social services.
- Coordinate with hospice, home health, and other medical providers to ensure continuity of care.
- Support residents in accessing ancillary and therapeutic services.
- Participate in care conferences and contribute to ongoing quality improvement efforts.
Support and Supervision
- Provide clinical support and mentorship to CNAs, Death Doulas, Resident Advocates and medical respite Volunteers.
- Ensure staff adhere to medication management, documentation, and emergency procedures.
- Serve as a resource for crisis intervention, triage, and compassionate care.
Documentation and Compliance
- Complete timely and accurate documentation in EMR and other systems.
- Report incidents, medication errors, and significant changes in condition to the DON and Administrator.
- Maintain confidentiality and compliance with HIPAA and Assisted Living regulations.
Qualifications
- Current LPN or RN license in good standing with the State of Utah.
- Nursing experience, preferably in assisted living, hospice, or medical respite settings preferred.
- Strong clinical assessment and critical thinking skills.
- Experience working with vulnerable or unhoused populations preferred.
- Excellent communication and teamwork abilities.
- Ability to work between the hours of 6:00pm and 6:00am with a mix of scheduled on-site and on-call work.
- Ability to work independently, remain calm under pressure, and exercise sound judgment during emergencies.

Health Requirements
- Health Evaluation performed by Director of Nursing including TB test and review of all immunizations.
- Must be current on flu and COVID-19 (including booster).
