Medical Case Manager

Customer Service Education/Training Human Services Outreach and Community Engagement

THRIVE’s Medical Case Manager (MCM) provides specialized medical case management services to individuals, families, or groups receiving services with our organization. MCM will also serve as our lead Community Health Worker and keep staff up to date on all public health information. The MCM is an integrated service position and works collaboratively with all staff, especially the Medical Director, to support the medical case management needs of clients.


Job Responsibilities:

60%

  • Direct Client Services: Maintain an active caseload of medically complex clients. Complete medical case management intakes with all clients on MCM caseload. Manage all referrals, health assessment coordination, and recommended follow up steps as assigned by Medical Director. Maintain all client charts for psychiatry-only clients, including up to date intakes and assessments. Regularly audit insurance/PCP/healthcare access issues to identify clients that are higher risk for losing coverage/or are having challenges with healthcare access. Advocate for clients including coordinating with insurance, billing offices, and clinics. Maintain up to date ROIs and assist Medical Director with obtaining/releasing records. Participate in actively soliciting feedback from program participants through distributing satisfaction surveys or other approved means. Meet or exceed program productivity expectations on a regular and sustained basis. Responsible for clinical documentation, i.e., progress notes, patient assessments, service/treatment plans, and other forms/records as required in a timely manner. Maintain detailed case files and other documentation in 100% compliance with program standards. Participate in clinical supervision, case consultations, program reviews and informal consultations as needed.

  • Coordination: Monitor and maintain up to date Salesforce referral lists for medical case management, health assessments, forensic evaluations, psychiatry clinic, and any related auxiliary services (massage) to organizational standards. Coordinate psychiatry clinic, ensuring all charts are up to date and clients are transferred back to their PCP for ongoing care when appropriate. Coordinating auxiliary services (internal or external)

  • Internal and External Collaboration: Consult and collaborate with THRIVE staff to meet client needs and overcome unique medical barriers. Support integrated relationship with community health clinics. Utilize external relationships to formally train all THRIVE staff on public health, programs that could benefit clients, and suggest new internal programming. When appropriate, advise leadership on project and partnership needs. Respond to inquiries from the community about THRIVE services.

  • Other Duties: Participate in other department and organization-wide activities, meetings and trainings. Complete administrative responsibilities. Perform other duties as assigned.



Required education, experience, certificates, licenses or registrations

  • Bachelor’s degree in public health or social services field or relevant work history equivalent
  • Minimum of 3 years of experience relevant to the position (public health, case management, vulnerable groups including refugees or immigrants)
  • Community health worker training available upon hire

Preferred education, experience, certificates, licenses or registrations

  • Valid driver’s license, access to an automobile, insurance, and willingness to drive to off-site locations

Competencies (Knowledge, Skills and Abilities)

  • Fluency in another language is preferred (Spanish, Arabic, Swahili, etc.)
  • Ability to work well with an interpreter
  • Experience, comfort, and sensitivity working in multi-cultural environment is required
  • Demonstrated understanding of the US immigration system, including asylum process
  • Excellent written, verbal and interpersonal communications skills
  • Ability to maintain confidentiality and HIPAA standards, recognize appropriate boundaries and cope with emotionally stressful situations
  • Ability to work as a part of an interdisciplinary team and multi-cultural team
  • Ability to work independently and effectively manage multiple priorities and one's own schedule
  • Flexibility to adapt to changing program requirements and political climate
  • Ability to work in a dynamic context and respond to various needs
  • Working knowledge of Microsoft Office suite (Outlook, Word and Excel) and Google Apps
  • Demonstrated interest in issues of immigrant communities, political torture, severe war trauma, social justice, and human rights issues
  • Must pass background check

Supervisory Responsibilities: No supervisory responsibilities

Work Environment: 

  • Services are provided on-site at THRIVE offices. Hybrid schedule is up to 5 hours per week for administrative tasks from home. Additional time away from office is likely, including accompaniment to medical appointments, attending or leading workshops, and serving as a representative of THRIVE at satellite locations such as medical clinics.
  • Time spent on the computer is approximately 75%

Physical Demands: While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to sit; stand; walk; use hands to finger, handle or feel; and reach with hands and arms. This position requires the ability to occasionally lift office products and supplies, of up to 10 pounds.

 

Travel: A valid driver’s license and insurance is required, and you may need to transport clients occasionally

About THRIVE Center for Survivors of Torture: The mission of THRIVE Center for Survivors of Torture (THRIVE) is to promote the health and well-being of the world’s torture survivors. At the core of torture, the trust between two human beings is destroyed. Our services focus on restoring trust and helping a survivor rebuild emotional, physical, social, and spiritual bonds. Our work also aims to prevent the effects of intergenerational trauma.

THRIVE was founded in 2003 in response to the unmet needs of torture survivors in our community. Today, THRIVE is the only organization in Utah that specializes in serving torture survivors. To date we have served over 4,500 individuals.

Our trauma-informed services include: individual, couple, & family psychotherapy • group therapy, support, &education • intensive case management • medical assessment, advocacy, & referral • psychiatric care •pharmacological education • legal referral for asylum seekers • massage therapy •preventative & wellness care • comprehensive assessment & evaluation • specialized training and consultation for professionals who encounter torture survivors in their practice.

We are looking for candidates who are passionate about human rights, mission-driven, self-motivated, committed to professional growth and learning and are excited about providing holistic services to refugees and asylum seekers.


Hours and Rate: 

  • Full-time, 40 hours per week. Can be adapted to part-time to meet the right candidate's needs.
    • Competitive pay and benefits, range reflective of a variety of backgrounds and licensures that the job description may be adapted to suit
    • Range is $23/hr-$26/hr
