Data Analyst

  • Create a monthly Productivity Report to track productivity metrics by collecting data from multiple sources to ensure accuracy for and consistency and provide findings to Leadership. Collaborate with Leadership as needed to ensure accuracy.
  • Perform exploratory data analysis to identify trends and patterns.
  • Create visually compelling dashboards, charts, graphs, and reports to present findings and communicate key insights.
  • Collaborate with cross-functional teams to understand business requirements and translate them into data analysis tasks.
  • Communicate findings as recommendations to Leadership through presentations, reports, and documentation.
  • Create the Annual Report for the CCBHC Grant, for Project Reality’s leadership, and make updates and adjustments as needed for other purposes.
  • Ad hoc data reporting from UWITS, Netsmart, and AdvancedMD that includes but is not limited to, patient records, nursing utility, patient population analysis, missed appointment reports, and former employee records.
  • Create and regularly maintain a Caseload Manager system ensuring that the data is accurate and up-to-date.
  • Manage the Employee Portal and make updates as needed.
  • Manage and Update the company’s website. Redesign and update the content and data as needed.
  • Assist in new software evaluations, implementations, and training.

Grant Evaluator

  • Attend and prepare data for the monthly CCBHC GPO meetings, the CCBHC Admin team meetings, and the CCBHC National Council meetings.
  • Assist leadership with SPARS tracking and manage the tracking sheet.
  • Regularly monitor the SPARS data to review performance, IPP and Health Indicators. Implement and update internal IPP tracking and monitor results to ensure they are in line with grant goals.
  • Report NOMs data into SPARS tracking system by quarterly deadlines as outlined by grant requirements.
  • Work with the grant PD and other leadership team members to write and manage required grant documents/reports including yearly continuation applications, annual programmatic reports, RAMs, and other grant documentation requirements that arise throughout the grant cycle.
  • Manage ERA Commons by managing grant documents, upload required grant documents, be informed of grant goals and deadlines as outlined in ERA Commons.
  • Be the point of contact for the State Opioid Response (SOR) Grant and attend meetings with the State Program Director. Respond to data requests and Coordinate with the Executive Director to complete assessments and reassessments. Update SOR tracker when new records are sent by the state.
  • Organize and manage Grant documents and track necessary components needed for application. Assist leadership by providing data and writing and organizing grant applications.
  • Work with Leadership and Stakeholders to develop and implement comprehensive evaluation frameworks for monitoring and evaluation.
  • Utilize treatment and recovery data to assess and effectiveness of programs, providing evidence of treatment and services effectiveness, and highlighting areas for improvement.
  • Develop summative and formative evaluation reports to demonstrate accountability to stakeholders and Leadership
