We are hiring a full-time Housing Support Coordinator for Salt Lake City, Utah!!

The Permanent Supportive Housing - Counselor works as a direct clinical services provider for civilian and Veteran residents of a 40-unit Permanent Supportive Housing Complex for individuals with chronic Substance Use Disorder. This position is responsible for individual sessions, life skills classes, group sessions, and case management services for clients suffering from chronic substance abuse and co-occurring disorders. All duties are to be performed within the SUDC scope of practice as defined by DOPL and Utah Law. Duties are performed under the clinical supervision of their assigned clinical training supervisor and other appropriately licensed supervisors. In addition, duties are performed under the management direction of the Outpatient Program Manager with input, direction, and collaboration from the Clinical Director and other members of the Executive Team.

  • Pay is $61,000 a year

Education and Experience

  • Experience in the substance-abuse field is preferred

Licenses/Certifications/Registrations

  • Substance Use Disorder Counselor licensure required

Job Duties assigned, but not limited to:

  • Assigned as client's secondary counselor, reviews all assessments available and initial treatment plan. As client's counselor, this person has the responsibility to coordinate some aspects of client's treatment and recovery efforts during residency. The counselor is responsible for reviewing admission to First Step House (FSH) informed consent, client fee agreement, and other relevant information. Counselor is familiar with and can convey to clients the process of intake, including paperwork and orientation; describing general philosophy and goals of FSH program; addressing housing costs to be borne by client and support goals to assist client with plan for payment; explains and reviews expectations governing client conduct and the rights of client.
  • Provides substance abuse counseling and coordinates services to establish support of client's family members and other entities, such as DCFS, AP&P, Courts, Vocational Rehabilitation, DWS, etc., as part of a comprehensive treatment plan.
  • Responsible for timely and correct Documentation in UWITS and paper file; includes Progress, miscellaneous interventions, crisis interventions, and circumstances that pertain to or provide support for client to accomplish treatment plans goals. Responsible for written and verbal reports, progress notes, client referrals and pertinent data.
  • Consults with other professionals, both internally and externally, regarding client treatment and services to assure comprehensive quality care for the client.
  • Attends clinical training classes, staff meetings and workshops as determined for job performance improvement.
  • Performs related duties as assigned by Training Supervisor, Clinical Services Manager and Director of Clinical Services.
  • Responsible for appropriate documentation of clinical and supervised hours as per professional, DOPL, scholastic institution, special training, or service provided.
